A 3-8% prepayment courtesy is allowed when fees in excess of $400 are paid in full by cash or check on the day that treatment is presented or in advance of scheduled appointment. 2. Interest-free and extended payment plans for as little as $50 a month for up to two years are available based on applicant's credit approval. 4.
